2 Quick Apply 1 hour ago Full Job Description Under the direction and supervision of the Chief Operating Officer, the Home Care Manager is responsible for the overall organization of the Pennswood at Home Program, establishing and maintaining procedures that enhance work flow throughout the department.
The Home Care manager acts as the liaison between the resident, resident's family, and Pennswood Village for the support of the resident through the Home Care department.
Act as liaison between resident, resident's family and Pennswood.
Review and ensure compliance with established policies and procedures, (Federal, State of Pennsylvania) for the Pennswood at Home Program.
Ensure plan of care is established for each resident and followed or modified as needed.
Maintain accurate and objective written and oral communication.
Communicate in a positive, professional and respectful manner with residents, family members and staff at all times.
Be an advocate for Pennswood at Home services with staff, residents, and families.
Ensure client satisfaction.
Problem-solve emergency situations.
Communicate concerns with an action plan to the C.

Coordinate staffing and scheduling and completing bi-weekly payroll.
Coordinate billing for PAH services.
Provide afterhours/weekend on call support to staff and residents.
Develop staff through orientation, training and management of staff performance.
Keep staff up-to-date on Pennswood Village department policies and procedures.
Ensure that all residents on service have completed a contract.
Review contract with resident and family as needed.
Provide support to other PV health care departments, providing services to support residents in those areas.
Work with electronic medical records.
Effectively coach the staff and ensure proper department training and completion of all Pennswood annual training requirements.
Participate in Transition meetings, discussing residents who may need a change in level within the Pennswood continuum.
Work as a team member with health services, social work, physician practice and other PV departments as required.
Participate in the development of the department operating and capital budgets, manage and monitor department expenditures within guidelines.
Maintain and ensure the personal rights of all patients through knowledge of and leadership in advocating the Patient's Bill of Rights.
Consistently seek to extend professional knowledge and improve skills.
Ensure proper staffing of department to meet and grow services, hiring staff as needed with support from HR.

Estimated Salary: $20 to $28 per hour based on qualifications.
